To arrange airport pick-up not included in your booking, please meet the 'Vismaya Staff' at the 2nd floor (passenger arrival floor), between gate number 3 and 4 at Suvarnabhumi Airport. The transfer is available for 80 Baht per person, per way.
Does the transfer include both a pick-up from the airport and a drop-off back to the airport?

Yes, the round trip transfer means you will be picked up from the airport to the hotel and also from the hotel back to the airport.
Is breakfast included with all room types, or is there an additional charge?
